1. Doctors of BC Practice Support Program Panel Dashboard

Purpose:

To integrate the PSP panel assessment reports within the OSCAR EMR to support the Understand Your Patient Panel program within physician practices.

The main objectives of this project are to:

1. Support physician practices with simplified and intuitive reporting functions that help facilitate the Understand Your Patient Panel program.
2. Introduce concepts, such as metric sharing, to support physicians in pursuit of clinical quality improvement and to provide measurable statistics to PSP for program management.
3. Train and deploy informed PSP resources to support physician efforts, with access to physician statistics to engage the physician’s clinic team (MOA) in the Understand Your Patient Panel program.

Status:

  • The PSP Panel Dashboard is currently being installed with upgrades to OSCAR V15.
  • Several enhancements to the Dashboard are being developed for delivery over the next several months.

2. Community Actions and Resources Empowering Seniors (CARES)

CARES is an evidence-based proactive model supporting active case finding, periodic Electronic Comprehensive Geriatric Assessments (eCGA) and integration with trained community health coaches. The model, which has been developed in collaboration between the Fraser Health Authority (FHA) and the province of Nova Scotia, is designed to enable seniors to live a higher quality of life within their community for longer, reduce emergency room visits and avoidable admissions to acute care and premature admission to residential care systems.

Purpose:

To support information management using CARES workflow and process methodology through integrating the CARES eCGA and associated health assessment tools within OSCAR. The project is being developed in two phases.
1) Phase 1: Develop technical specifications and a project proposal to implement eFI-CGA into OSCAR
2) Phase 2: Implement eFI-CGA into OSCAR

Status:

  • Phase 1 has been completed and submitted to the Fraser Health Authority for consideration.
