Did you create an eForm or Report by Template (RBT) and want to share it publically so other OSCAR users can benefit from it? The OSCAR community loves sharing knowledge and helpful resources such as eForms & RBT’s, so why not help others save time by sharing your eForm / RBT to the Oscargalaxy.org downloads repository  (the largest OSCAR downloads repository in Canada).

PLEASE READ before submitting your request:

•  After verifying that your completed eForm or RBT does not already exist in the downloads repository,  submit it to us using the form below with the details of your eForm or RBT.  If the content you submit already exists in the repository, it will not be published (no duplicates permitted).  
• Your eForm or RBT will be tested for security and functionality, and if viable, uploaded to the repository.  
• Proper attribution is provided when the content is published, so make sure to include your name (the listing’s description will include “Shared by  <Your Name>). 
• Please keep the name of your file simple.  We reserve the right to modify requested listing names, to avoid non-duplication or confusion, and ease search parameters. 
• We do not publish PDF (fillable or not) or Word documents.  We also don’t edit PDFs or Word documents.
• There is no cost to share a fully prepared and tested eForm or RBT, but we do charge fees to create a new or to update / edit an eForm.   
• If you change anything in the original PDF form that effects the layout or content of the eForm after it has been uploaded for sharing, you will either have to reprogram the changes to the eForm yourself and resubmit the full .zip packet as a replacement, or pay a fee for us to make the changes.  
• If you would like us to program your form from PDF to eForm, please use the “Order eForms” request.

*eForms (with all components) must be submitted in a .zip file.

*RBT’s must be submitted as a .txt format file.
